If you are employed by a not-for-profit organization you may be eligible for the Public Service Loan Forgiveness (PSLF) Program. This includes physicians who provide care or other services in a clinic, facility or hospital, owned or operated by a non-profit organization, such as Hoag Clinic (Clinic) and/or Hoag Memorial Hospital Presbyterian (Hospital).
The PSLF Program forgives the remaining balance on Direct Loans after the equivalent of 120 qualifying monthly payments under an accepted repayment plan, while working full-time for an eligible employer.
The following information is required to complete the PSLF form:
- Employer email address: hr@hoag.org
- Employer Identification Number (EIN)
- The appropriate Hoag entities are listed below, each with a corresponding Employer Identification Number (EIN). Please be sure to enter the correct entity and EIN to prevent delays.

Once you complete the Borrower information, the form will be routed via DocuSign to Human Resources who completes the Employer Certification and electronically submits it to the PSLF servicer for processing.
While a representative of the Clinic or Hospital will sign the Employer Certification, we are not able to determine whether any particular individual will qualify for the PSLF program. Please review the PSLF FAQs for further information, and seek guidance from your loan servicer or directly contact the Department of Education through their provided hotlines or emails to inquire about eligibility.
